The Care Manager is directly responsible to her/his supervisor, who assigns cases, reviews case management and administrative work, and provides supervision and consultation. The Care Manager is expected to carry out routine assignments independently within the Agency’s program policies and procedures and within her/his scope of responsibility. This role involves managing a full caseload of over 50 Holocaust survivor clients, providing direct casework, conducting intake and needs assessments, formulating and implementing service plans, and evaluating service outcomes. The Care Manager will also perform related activities such as collaborating with other agencies, utilizing JSSA’s resources, providing information on community resources, and acting as a client advocate. Documentation of all service provision in Unicentric, maintaining current and accurate records, case recordings, and statistics, and ensuring client confidentiality are crucial aspects of the role. The position requires establishing close working contacts in the community and participating in department staffing, case conferences, and program-related meetings.
