Care Manager - Ottawa (Full-Time with Benefits)
About The Position
INTEGRAcare is seeking an enthusiastic, dedicated, and exceptional RPN to join its Care Coordination team. The Care Manager is responsible for providing primary care management, nursing support, and coordinating & scheduling care for Clients to ensure they maintain their dignity, independence, and joyful living. This role offers the opportunity to work in one of the leading Home Care companies in Canada, learn new skills related to Client and Caregiver management, and manage the care of valued INTEGRAcare Clients. Responsibilities include developing and maintaining dynamic care plans and schedules, leading/coaching Caregiver teams, interacting with a multidisciplinary team, and teaching new Graduate PSWs various elements of client care. Requirements
- A valid CNO Registration in good standing
- Be Bilingual (English/French)
- Have a great attitude and willingness to work in a fast-paced environment
- Strong Attention to Detail and desire to “Close the Loop”
- Possess good written and verbal communication skills
- Be passionate about providing our Clients and Caregivers with the highest quality care and support
- Be open to new learning opportunities, be coachable and dependable
- Have a valid driver’s license in good standing
Nice To Haves
- 1-2 years of Nursing experience (preferred)
Responsibilities
- Manage your own INTEGRAcare Clients including establishing and modifying dynamic care plans and schedules
- Manage Care Teams by planning and scheduling Caregivers with a particular focus on continuity and consistency of care for your Clients
- Help teach the new hires program and assist in the orientation process of new hires
- Be part of the on-call rotation
- Participate in company and team building events
- Developing and maintaining dynamic care plans and schedules for our Clients
- Lead/coach teams of Caregivers and interact with our multidisciplinary team (Physiotherapists, massage therapists, doctors etc.)
- Teach New Graduate PSWs various elements of caring for our Clients
