As a Care Manager in our PACE program, you are the primary coordinator for our participants' care experience. You serve as their advocate, navigator, and the person responsible for making sure their care plan comes to life. Your work ensures participants receive the right services across all settings while maintaining their independence and quality of life. This role demands exceptional attention to detail, strong communication skills, the ability to track and synthesize information across multiple moving pieces, and persistent, diligent follow-through. You will work across a wide range of stakeholders — including participants and their families, our interdisciplinary team, PACE leadership, external care coordinators, authorization teams, and partners in DME, pharmacy, and specialist care — and will be relied upon to keep everyone informed, aligned, and moving in the right direction.

Requirements

  • 2–5 years of experience in healthcare coordination, care management support, health operations, or a similar role — or equivalent demonstrated capability
  • Demonstrated ability to manage across multiple systems and communication channels simultaneously — EHRs, task management tools, messaging platforms, and more
  • Comfort working in fast-moving, resource-constrained environments — startup, community health, high-volume clinic, or similar
  • Experience evaluating or implementing new tools and workflows in a healthcare or operations setting is a plus
  • Mandarin or Cantonese language proficiency a significant plus
  • Background in social work, public health, community health, or health administration a plus — but not required
  • Clinical licensure (LVN, CNA, MA, SW, etc.) welcome but not required
  • Los Angeles required. Ability to commute to our Alhambra center required.

Responsibilities

  • Serve as a coordination hub for participant care — tracking follow-up needs, managing transitions, and ensuring the clinical team has the information and support they need to act.
  • Coordinate care transitions, including post-hospitalization follow-up: arranging DME, scheduling PCP follow-ups, facilitating referrals, and flagging care plan update needs to the appropriate licensed team members.
  • Manage day-to-day communication and logistics across care settings — home health agencies, specialists, facilities, and internal staff — to ensure continuity and timely service delivery.
  • Support care team meeting preparation and follow-through: organizing agendas, tracking action items, flagging participant status changes, and helping the team stay on top of what needs to happen next.
  • Maintain accurate, up-to-date documentation across platforms — tracking service coordination activities, participant status, and utilization data in our electronic health record and other systems.
  • Use technology tools fluently and creatively to organize workflows, communicate across teams, and surface the right information at the right time — we rely on a variety of platforms and expect you to get up to speed quickly and help us use them better.
  • Evaluate and adopt new tools as our tech stack evolves; help identify where technology can reduce friction in our care coordination workflows.
  • Build relationships with participants and families, serving as a trusted point of contact and communications bridge — with cultural and language sensitivity central to how you work.
  • Identify gaps in coordination or follow-through and proactively propose solutions; contribute to building the processes and workflows that make this function scale.
