About The Position

The Autism Society of North Carolina is hiring a full-time Care Manager to support individuals with autism and developmental disabilities through compassionate, community-based care. Requirements

• Someone who meets QP qualification in the state of NC
• Bachelor’s or master's in human services (Social Work, Psychology, etc.)
• 2+ years' experience with Autism and/or Intellectual /Developmental Disabilities
• 2+ years’ experience providing care management, case management, or coordination to individuals with IDD/ASD
• Based in or near the Triangle area and willing to travel locally
• Strong communication, organization, and problem-solving skills
• Self-starter who thrives in a collaborative environment

Responsibilities

• Provide whole-person care management for individuals and families
• Collaborate with care teams, service providers, and families
• Conduct assessments and develop personalized Plans of Care
• Submit referrals, manage authorizations, and monitor service delivery
• Ensure compliance with internal and external standards
