About The Position

Westchester Jewish Community Services (WJCS) is seeking a compassionate and organized full-time (35 hours per week) Care Manager to join our Home Delivered Meals Program. In this role, you will offer services, including in-home assessments, case management, care planning, information and referrals to the seniors participating in the program. The salary is $45,000.

Requirements

• Bachelor’s Degree in a human services field
• Experience working with senior citizens
• Valid driver’s license and vehicle
• Computer literacy

• Bilingual English/Spanish is a plus

Responsibilities

• Conduct comprehensive in-home assessment, including financial assessments, to determine eligibility for the grant, programs, services and entitlements.
• Complete home visits throughout the Westchester County community.
• Conduct annual reassessments, as well as reassessments whenever necessary.
• Develop individualized client Care Plans.
• Ensure clients and their caregivers receive information about and assistance with appropriate referrals.
• Maintain accurate, timely and complete documentation in accordance with agency program requirements.
