About The Position

Hamilton-Madison House’s Caregiver Support Program serves informal/unpaid caregivers of older adults, and their loved ones, throughout Manhattan’s community districts 1-7, comprising all of Manhattan below 59th Street and the west side of Manhattan to the northern tip of central park. Care Managers conduct outreach, telephone intakes and in-person assessments and reassessments, counseling, screen clients for eligibility and assist them in apply for benefits and entitlements, provide information and referral services, develop and implement client care plans, including Respite and Supplemental Services and coordinate or provide caregiver training, and support groups.

Requirements

  • Master’s Degree in Social Work or related field preferred with one year experience in case work or counseling (MSW internship included) or Bachelor’s Degree in Social Work or related field with 2 years’ experience in care management or counseling (Internship included)
  • Excellent engagement skills, including telephone skills, demonstrate empathic listening and the ability to develop and maintain interpersonal relationships MSW/BSW or related degree.
  • Nonjudgmental, socially perceptive, empathic and a good listener.
  • Sensitivity to understanding the needs of caregivers and older adults.
  • Knowledge of client engagement, assessment, and the biological, psychological, and social aspects of aging and the impact of disabilities and illness on aging and the family.
  • Awareness of types of benefits and entitlements and experience completing application forms, especially related to older adults.
  • Highly organized and detail oriented; able to plan, manage and successfully complete multiple concurrent situations.
  • Ability to handle complex and difficult situations in a professional manner. Demonstrated ability to conduct oneself in a calm and professional demeanor when dealing with the public and/or with difficult situations.
  • Engaging and Strong interpersonal skills demonstrative of empathy and support.
  • Willingness to make community visits and take initiative.
  • Strong written and verbal communication.
  • Understanding and appreciation of the role of race, religion, ethnicity, sexual preference and individual values as it relates to serving clients and their families. Commitment to diversity and ability to work with diverse population of seniors, clients, volunteers and staff.
  • Ability to travel on public transportation and climb stairs to conduct home visits and information sessions.
  • Basic computer literacy and Microsoft Windows and Microsoft Office.
  • Ability to work independently as well as on interdisciplinary teams.

Nice To Haves

  • Knowledge of Vive client databases.
  • Bilingual preferred: English/Cantonese or English/Spanish or English/French Creole or English/Other.

Responsibilities

  • Initiate community engagement, especially in collaboration with Older Adult Service Providers.
  • Maintain client caseload of ongoing and short-term clients/caregivers.
  • Conduct telephone intake interviews.
  • Conduct home visits to assess caregiver and care recipient needs using standard assessment forms and documentation of ADL and IDL needs.
  • Use standard screening tools to assess clients for benefits/entitlements eligibility and assist them in completing and submitting forms and following up to ensure benefits/entitlements are received.
  • Utilize support care planning for the caregiver and coordinate referrals to appropriate resources.
  • Maintain case records on database including case notes and documentation of services provided.
  • Ensure timely follow-up on caregiver contacts and assessments and reassessments.
  • Assist in implementing caregiver training workshops and caregiver support groups for caregivers in their role and help them maintain their own health and wellbeing.
  • Promote the organization and caregiver program through outreach, participation in community events, tabling and representing the organization to the public.
  • Participate in interdisciplinary team case conferences and review and implement programming to benefit the Hamilton-Madison House’s overall caregiver services department.
