About The Position

Crossroads Treatment Centers is seeking a Part-time Care Manager to join their team. This role involves connecting with patients monthly to address their needs and provide necessary resources, as well as maintaining relationships with community partners. The Care Manager will work closely with a dedicated team to provide exceptional patient care and participate in community awareness events to promote Crossroads and its mission. Crossroads Treatment Centers has been a leader in treating patients with opioid use disorder since 2005, offering accessible, high-quality, evidence-based medication-assisted treatment (MAT) options. Their comprehensive approach is the gold standard in care for opioid use disorder, proven to prevent overdose deaths and lead to long-term recovery. They are committed to improving access to treatment across the U.S., serving over 26,500 patients through outpatient, office-based clinics in multiple states.

Requirements

At minimum, High School Diploma or GED required.
Knowledge of compliant documentation practices

Nice To Haves

Experience in a fast-paced environment, servicing many clients, is preferred.
Experience in healthcare, addiction medicine, community health, care coordination preferred.
Experience with building individualized care plans is a plus.
Lived Addiction experience is a plus.
