About The Position

Liberty Resources is seeking an Adult Health Home Care Manager Team Lead for Onondaga County. This position is full-time, Monday through Friday, 40 hours per week during regular business hours. The position operates in a hybrid work model with required travel to conduct home and community-based visits. The Team Lead will function as part of the leadership team to assist the Supervisor with oversight of Care Managers. The Team Lead will be responsible for coaching, collaborating with care managers and providers, and when possible, educating the member and other care team members of the health benefits of closing identified gaps. Closures in critical Gaps in Care can make a significant impact on an individual’s life including improved health, a decrease in avoidable hospitalizations and emergency usage, and overall improved quality of life for the individual.

Requirements

  • Bachelors of Science or Arts w/2 yrs. relevant experience; Registered Nurse with w/2 yrs. relevant experience; or Masters w/1 yr. relevant experience.
  • Must have a valid New York State driver’s license and access to reliable transportation.

Responsibilities

  • Assists Supervisor with coaching and training of Care Management Staff
  • Shadows one appointment per month with each Care Manager on their team to provide coaching and ensure quality services are rendered
  • Serves as a mentor to onboarding staff to provide additional support to new staff members
  • Attends Health Home Hub meetings and provides feedback to leadership team
  • Facilitates team meetings on a monthly basis to offer training and coaching on policies and program standards
  • Identifies training needs for team and implements training and coaching for staff
  • Maintains a small case load of active clients
  • Assists with enrollment activities as needed to be assigned to Care Managers upon enrollment
