About The Position
The RN Care Coordinator is an experienced RN who functions as a core member of a multidisciplinary team and communicates patient's health care needs with primary care, behavioral health services, health care facilities and/or payers. The Care Coordinator is responsible for coordinating and supporting care within the residency clinic and for coordinating referrals to other clinically based services outside of the clinic. The Care Coordinator may provide evidence-based treatments or work with other mental health providers when such treatment is indicated. Requirements
- Graduate from an accredited school of Nursing
- Washington Registered Nurse License
- 3 years clinical nursing experience.
- 1 year Care Management or similar experience.
