About The Position

CareFor is seeking a skilled, compassionate Registered Nurse (RN) to join our care management team, supporting clients in their homes and across the community. This is a high-touch, relationship-driven role focused on coordinating care for primarily (but not exclusively) older adults with medically complex needs. Unlike traditional home health, our care managers oversee the full picture, advocating for clients, guiding families, and collaborating across medical and non-medical teams. This role is ideal for an experienced nurse who values autonomy, critical thinking, and meaningful client relationships.

Requirements

  • Active Registered Nurse (RN) license in Texas (or eligibility via reciprocity)
  • Bachelor of Science in Nursing (BSN) or higher
  • Minimum of 5 years of clinical nursing experience
  • Residence within the Austin metro area (Georgetown to San Marcos)
  • Valid driver’s license, reliable vehicle, and ability to meet insurance requirements
  • Strong clinical judgment with the ability to assess and respond to complex situations
  • Excellent documentation and report-writing skills with high attention to detail
  • Highly organized and able to manage multiple clients independently
  • Confident decision-maker who seeks guidance when appropriate
  • Strong interpersonal and communication skills across diverse populations
  • Ability to balance autonomy with collaboration in a team environment
  • Adaptable to changing client needs, environments, and priorities
  • High level of integrity, professionalism, and discretion with sensitive information

Nice To Haves

  • 10+ years of diverse nursing experience
  • Background in hospice, home health, care management, or similar roles
  • Experience with medically complex or geriatric populations
  • Prior experience coordinating care across multiple providers or settings

Responsibilities

  • Conduct comprehensive client assessments (medical, social, environmental) and reassess as needed
  • Develop, implement, and continuously evaluate individualized care plans
  • Coordinate care across a multidisciplinary team (physicians, therapists, home health, hospice, caregivers, and community providers)
  • Serve as a trusted resource for clients and families; educating, advocating, and guiding care decisions
  • Write detailed clinical documentation, including assessments, progress notes, and medical necessity reports
  • Monitor client progress and adjust care plans to ensure goal-driven, patient-centered outcomes
  • Accompany and/or transport clients to appointments or procedures as needed, advocating for their care in real time
  • Collaborate with internal teams (staffing, accounting, leadership) to support overall client care
  • Support caregivers with guidance, communication, and care direction
  • Participate in guardianship responsibilities when applicable, coordinating with families, attorneys, and trust officers
  • Manage all administrative responsibilities independently, including documentation, communication, and record-keeping
  • Respond in person to client needs during business hours across the Austin metro area
