About The Position

The Patient Care Resource Manager (PCRM) is a nurse case manager responsible for facilitating patients through the healthcare system. This includes pre-admission through post-discharge coordination of care, resources, and services using the nursing process. The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput, improve care quality and efficiency, and ensure efficient resource utilization. The PCRM identifies and removes barriers to the progression of the plan of care for each patient. The PCRM collaborates with physicians, nurses, and other healthcare workers to manage the patient's transition to the next level of care. The PCRM serves as the central information source for third-party payers, communicating the plan of care and medical necessity, and gaining approval for post-discharge care. Through the use of evidence-based criteria, the PCRM assists physicians in ensuring the appropriate level of care for each patient.

Requirements

  • Bachelor’s degree
  • RN licensure required
  • Five years recent clinical experience applicable to the population preferred
  • Knowledge of clinical practice guidelines, computer data bases, and payer protocols desired

Responsibilities
