About The Position

The Care Manager Registered Nurse supports Aetna’s care management program for members enrolled in Medicare and Medicaid who have complex health and social needs. The nurse collaborates with members, providers, and community organizations to address healthcare and social determinant needs. Using critical thinking and judgment, the nurse facilitates appropriate healthcare outcomes by helping members schedule appointments, access benefits, and obtain education through care management tools and resources. This is a work-from-home role for an Illinois resident, with field travel of up to 75% of the time in and around Metro East and Southern Illinois. The role requires a private, secure home workspace and compliant internet connection. CVS Health provides computer equipment; the employee supplies workspace furniture. Mileage is reimbursed under the company expense reimbursement policy.

Requirements

Must reside in the state of Illinois.
Must have reliable transportation and be willing and able to travel up to 75% of the time in and around Madison, St. Clair, Monroe, Clinton, Jersey, Calhoun, Bond, Macoupin, and surrounding areas; mileage is reimbursed under the company expense reimbursement policy.
Minimum 3–5 years of clinical practical experience.
Confidence working from home as an independent thinker and using tools to collaborate and connect with teams virtually.
Excellent analytical and problem-solving skills.
Effective communication, organizational, and interpersonal skills.
Ability to work independently.
Effective computer skills, including navigating multiple systems and keyboarding.
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, and some proprietary applications.
Associate's degree required.
Active, unencumbered Registered Nurse license in the state of Illinois.
Must have or be able to obtain a direct, hardwired internet connection to a modem/router within 7 feet of the computer, with minimum speeds of 25 Mbps download and 3 Mbps upload; Wi-Fi and satellite internet are not permitted.
Must maintain a quiet, secure, private, designated home work location that is free from distractions, organized, compliant with CVS Health and HIPAA guidelines, and suitable for uninterrupted work.
Must work within the state and city where the employee has confirmed they currently live.
Must supply workspace furniture, such as a desk or standing desk, and adhere to the stated work-from-home requirements.
Comfort using multiple monitors and navigating multiple applications simultaneously.
Ability to use digital communication channels, including email, calendar invites, Teams messaging, and virtual meetings.
Experience with Microsoft Office 365 applications or similar tools such as Google Workspace.
Ability to access secure systems such as VPN and EHR portals, secure unattended devices, manage strong passwords, and recognize suspicious emails or links.
Ability to independently resolve common technical issues and contact IT for issues that cannot be resolved.
Openness to learning new skills as the workplace environment evolves.

Nice To Haves

2–3 years of care management, discharge planning, and/or home health care coordination experience.
Certified Case Manager credential.
Bilingual.
Bachelor's degree preferred.
