Care Manager Registered Nurse - Field (Metro East/Southern IL)
About The Position
The Case Management Registered Nurse uses critical thinking and judgment to collaborate with and inform the case management process. The nurse facilitates appropriate healthcare outcomes for members by helping with appointment scheduling, identifying and accessing benefits, and providing education through care management tools and resources. The position serves members enrolled in Medicare and Medicaid who may have complex health and social challenges, collaborating with members, providers, and community organizations to address health care and social determinant needs. The role is based from home in Illinois and requires travel up to 75% of the time in and around Metro East and Southern Illinois. The employee must maintain a private, secure, quiet home workspace and meet the specified wired internet requirements. CVS Health provides computer equipment, while the employee supplies workspace furniture. The position is full-time, with 40 anticipated weekly hours.
Requirements
- Must reside in Illinois and work from the state and city where currently living.
- Must have reliable transportation and be willing and able to travel up to 75% of the time in and around Metro East/Southern Illinois.
- Minimum 3–5 years of clinical practical experience.
- Confidence working independently from home and using tools to collaborate with teams virtually.
- Excellent analytical and problem-solving skills.
- Effective communication, organizational, and interpersonal skills.
- Ability to work independently.
- Effective computer skills, including navigating multiple systems and keyboarding.
- Proficiency with standard corporate software, including MS Word, Excel, Outlook, and PowerPoint, and some proprietary applications.
- Active, unencumbered Registered Nurse license in Illinois.
- Associate's degree required.
- Must have or be able to obtain a direct, hardwired internet connection to a modem/router within 7 feet of the computer, with minimum speeds of 25 Mbps download and 3 Mbps upload; Wi-Fi and satellite internet are not permitted.
- Must maintain a quiet, secure, private, tidy, and organized home workspace compliant with CVS Health and HIPAA guidelines, allowing uninterrupted work during work hours.
- Must be comfortable setting up and using multiple monitors and navigating multiple applications simultaneously.
- Must be able to communicate using email, calendar invites, Teams messaging, and virtual meetings.
- Must be able to access secure systems such as VPN and EHR portals, follow computer and password security practices, and recognize suspicious emails or links.
- Must be able to troubleshoot common technical issues independently and contact IT for unresolved issues.
- Must be open to learning new skills as the workplace environment evolves.
Nice To Haves
- Bachelor's degree.
- 2–3 years of care management, discharge planning, and/or home health care coordination experience.
- Certified Case Manager.
- Bilingual.
