About The Position

The Care Manager Registered Nurse supports Aetna’s care management program for members enrolled in Medicare and Medicaid who have complex health and social needs. The role collaborates with members, providers, and community organizations to address healthcare and social determinant needs and facilitate appropriate healthcare outcomes. Responsibilities include assisting members with appointment scheduling, identifying and helping them access benefits, and providing education using care management tools and resources. The position is work-from-home, with required travel up to 75% from the employee’s home location in and around the West Suburbs of Illinois, including DuPage, Kane, DeKalb, Kendall, and surrounding areas. Mileage is reimbursed under the company expense reimbursement policy. The role requires a quiet, secure, private home workspace and a direct wired internet connection meeting specified speed requirements. CVS Health provides computer equipment and related peripherals; the employee supplies workspace furniture. The position is scheduled for 40 hours per week and is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to base pay.

Requirements

Must reside in Illinois and work from the state and city where the employee has confirmed they currently live.
Must have reliable transportation and be willing and able to travel up to 75% from the home location in and around DuPage, Kane, DeKalb, Kendall, and surrounding counties in Illinois.
Minimum 3–5 years of clinical practical experience.
Active, unencumbered Registered Nurse license in Illinois.
Associate’s degree required.
Ability to work independently and confidence working from home as an independent thinker while collaborating and connecting with teams virtually.
Excellent analytical and problem-solving skills.
Effective communication, organizational, and interpersonal skills.
Effective computer skills, including navigating multiple systems and keyboarding.
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, and some proprietary applications.
Must have or be able to obtain a direct/hardwired internet connection to a modem/router within 7 feet of the computer, with minimum speeds of 25 Mbps download and 3 Mbps upload; Wi-Fi and satellite internet are not permitted.
Must maintain a quiet, secure, private, organized home work location that complies with CVS Health and HIPAA guidelines and allows uninterrupted work during work hours.
Must provide workspace furniture, such as a desk or standing desk, and be able to remain at a desk for extended periods.
Comfort using multiple monitors and applications simultaneously.
Ability to use secure systems such as VPN and EHR portals and follow security practices.
Ability to learn new skills as the workplace environment evolves.

Nice To Haves

Bachelor’s degree preferred.
2–3 years of care management, discharge planning, and/or home health care coordination experience.
Certified Case Manager credential.
Bilingual.

Responsibilities

Navigate multiple applications and systems and communicate using digital channels, including email, calendar invites, Teams messaging, and virtual meetings.
Follow secure systems and work-from-home practices, including managing passwords, locking unattended screens, and identifying suspicious emails or links.
Resolve common technical issues independently and contact IT for unresolved technical issues.
