About The Position

The Case Management Coordinator uses critical thinking and judgment to collaborate with and inform the case management process. The coordinator facilitates appropriate healthcare outcomes for members by assisting with appointment scheduling, helping members identify and access benefits, and providing education through care management tools and resources.

The role serves members enrolled in Medicare and Medicaid who may have complex health and social challenges. It involves collaborating with members, providers, and community organizations to address members’ healthcare and social determinant needs. The position is home-based, requires residence in Illinois, and involves travel in and around Kane and DuPage Counties and surrounding areas. The role is anticipated to require 40 hours per week.

Requirements

• Must reside in Illinois and work within the state and city where currently living.
• Active and unencumbered Registered Nurse license in Illinois.
• Associate's degree required; bachelor's degree preferred.
• 3–5 years of clinical practical experience.
• Reliable transportation and willingness and ability to travel up to 50–75% of the time in and around Kane and DuPage Counties and surrounding areas; mileage is reimbursed under the company expense reimbursement policy.
• Ability to work from home and think independently, using tools to collaborate and connect with teams virtually.
• Excellent analytical and problem-solving skills.
• Effective communication, organizational, and interpersonal skills.
• Ability to work independently.
• Effective computer skills, including navigating multiple systems and keyboarding.
• Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, and some proprietary applications.
• Ability to obtain and maintain a direct, hardwired internet connection to a modem/router within 7 feet of the computer, with at least 25 Mbps download and 3 Mbps upload speeds; Wi-Fi and satellite internet are not permitted.
• A quiet, secure, private, organized home work location that complies with CVS Health and HIPAA guidelines and allows uninterrupted work during work hours.
• Ability to set up and use multiple monitors and navigate multiple applications simultaneously.
• Ability to communicate through email, calendar invites, Teams messaging, and virtual meetings.
• Experience with Microsoft Office 365 applications or similar tools such as Google Workspace.
• Ability to access secure systems such as VPN and EHR portals, secure the computer when unattended, manage strong passwords, and recognize suspicious emails or links.
• Ability to resolve common technical issues independently and contact IT for issues that cannot be resolved.
• Commitment to meeting internet and other work-from-home requirements if hired.
• Must provide personal workspace furniture, such as a desk or standing desk.

Nice To Haves

• 2–3 years of care management, discharge planning, and/or home health care coordination experience.
• Certified Case Manager.
• Bilingual.
• Bachelor's degree.
