The Care Manager Registered Nurse (RN) GCM is responsible for facilitating communication among patients/families, multidisciplinary teams, medical management teams, and community resources to identify and resolve barriers to care delivery. This role involves understanding and interpreting contracts to achieve optimal clinical and financial outcomes. The Care Manager RN collaborates with patients, families, and inter-professional teams to ensure quality, efficient, and cost-effective healthcare services, potentially working embedded in a provider office or telephonically. They utilize evidence-based approaches to increase patient and family activation and engagement, developing SMART goals and assisting with patient self-management educational resources. The role also involves identifying potential learning barriers, reporting abnormal findings, and collaborating on care plans. The Care Manager RN independently manages their caseload, ensuring timely completion of tasks and documentation for MCO, regulatory, and contractual requirements. They partner with at-risk patients throughout their care journey, anticipating needs, responding to status changes, and prioritizing care based on essential patient needs. Additionally, they coordinate patient information and communication between various entities to ensure smooth transitions of care and coordinate referrals to internal and external resources as needed. This position requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development across the lifespan and assessing patient status based on age-specific needs.
