This specific role is for an LPN in Missouri, based in the Greater St. Louis area, requiring frequent travel to patient homes. The position is part of the In-Home Services (IHS) model, a Medicaid program supporting independent living through in-home assistance. The Care Manager, LPN is crucial for conducting in-home assessments, providing ongoing care management, and ensuring person-centered care. This role involves clinical assessment, care coordination, patient/caregiver education, and quality assurance. The LPN will collaborate with patients, caregivers, and care teams to assess needs, adjust care plans, and facilitate service delivery. Regular home visits will be conducted to evaluate well-being, monitor care quality, and identify opportunities for improvement in health outcomes, safety, and compliance. Proactive communication of clinical concerns and changes in condition to relevant team members is essential for timely intervention and optimal patient outcomes.

Requirements

Diploma from an accredited nursing program.
Valid Licensed Practical Nurse license and in good standing by Missouri Board of Nursing
Minimum of (1) year of experience with the care of the elderly, or individuals with disabilities or medically complex conditions.
Valid driver’s license and car insurance required, with means of reliable transportation.

Ability to follow established scripts, workflows, policies, procedures, quality standards, and regulatory requirements.
Strong active listening skills, with the ability to understand patient needs, clarify concerns, and provide the necessary support and escalation when appropriate.
Effective verbal and written communication skills, with the ability to clearly summarize interactions, document case activity, and communicate case status and next steps.
Strong typing and computer proficiency, with the ability to efficiently navigate contact center technology, customer relationship management systems, telephone platforms, and other necessary software applications while accurately documenting interactions.
Effective problem-solving and critical thinking, with the ability to evaluate information, identify appropriate solutions, exercise sound judgment, and escalate concerns when necessary.
Dependable and punctual, with a demonstrated commitment to consistent attendance, schedule adherence, and performance expectations.
Ability to adapt to changing priorities, processes, systems, and business needs in a fast-paced environment.

Responsibilities

Conduct initial in-person clinical assessments and coordinate timely service activation, ensuring all required documentation, approvals, and standard procedures are completed.
Identify potential changes to the individualized plans of care, making and documenting revisions as the patient’s needs or condition change.
Educate patients, caregivers, and internal teams regarding care needs, safety considerations, service requirements, caregiver responsibilities, and next steps.
Coordinate patient care through timely communication and documentation with physicians, clinical disciplines, internal partners, and appropriate community resources.
Provide ongoing education and coaching to support effective use of established best practices, promote caregiver competency, and connect patients and families with appropriate resources.
Review participant service records regularly and respond appropriately to identified changes in the patient’s condition, environment, or service needs.
Identify changes in condition, safety concerns, service gaps, incidents, or other urgent issues and escalate them in accordance with established clinical protocols.
Complete and document assessments, visits, clinical observations, care-plan updates, incidents, communications, and follow-up actions accurately, thoroughly, and within required timeframes.
Support authorization, renewal, start-of-care, and continued-service processes, as applicable.
Maintain compliance with all applicable regulatory requirements, professional standards, and company policies and procedures.
Perform other related duties as assigned by the supervisor.
