About The Position

This position is within the Protective Services for Adults program at Catholic Charities of Broome County. The Care Manager Level 2 will be responsible for providing direct social work services to individuals with special needs, including those living with mental illness, developmental disabilities, substance use issues, or the elderly. The role involves case management, crisis intervention, and coordination with various service providers.

Requirements

  • Minimum associates degree in human services or related field.
  • Minimum of one year experience for bachelor’s degree and two years for associates degree working in providing direct social work service to special needs individuals including those living with a mental illness, developmental disability, substance use or elderly.
  • Knowledge of community mental health services and services available to developmentally disabled and geriatric.
  • Excellent communication skills, both oral and written.
  • High degree of organizational skills to meet assigned work demands.
  • Demonstrated ability to work with a team as well as function independently.
  • Good computer skills with working knowledge of Microsoft Office necessary.
  • Knowledge of special needs individuals including, mental illness/psychiatric disorders/developmentally disabled and geriatrics.
  • The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs. or less.
  • A valid driver’s license and the ability to legally operate a vehicle in NYS.
  • Regular and substantial contact with those individuals receiving services.
  • Physical condition commensurate with the demands of the position.

Nice To Haves

  • Bachelor’s degree in a human services field is preferred.

Responsibilities

  • Assist recipients in learning to use financial resources responsibly, including assistance with budgets.
  • Provide crisis intervention services as necessary.
  • Promote self-advocacy and personal empowerment.
  • Maintain case records, progress notes and assessments, as required in compliance with Agency, County and Office of Children and Family Services regulations.
  • May carry caseload of up to 40 recipients and provide a minimum of one visit per month.
  • Complete and submit monthly reports related to services provided and individuals served.
  • Encourage medication, education and compliance.
  • Communicate regularly with other providers and individuals to develop or enhance consistency of services.
  • Coordinates Representative Payee services as needed.
  • Participate in ongoing training, attend meetings as required.
  • Perform other related duties as required.
