Care Management is a collaborative practice model including the patients, nurses, Care Manager LCSWs, physicians, other practitioners, caregivers and the community. The care management process encompasses excellent communication, both verbal and written, and facilitates care along a continuum through effective resource coordination and addressing the psych social needs of the patient. Requirements

• Master's degree in social work
• California Clinical Social Worker License
• 2 years of Relevant Social Work experience in a medical group, health plan, hospital, community, post-acute, or medical office/clinic setting.

Nice To Haves

• 3 years of Care management and Managed Care experience in acute, ambulatory, or community health setting.
• 3 years of Behavioral Health experience in acute or outpatient setting.

Responsibilities

• Address the psych social needs of the patient.
• Facilitate care along a continuum through effective resource coordination.
