About The Position

The Housing Care Manager will oversee resident support and documentation for individuals placed in Short-Term Post-Hospitalization and Recuperative Care housing. This is a hands-on coordination role that requires strong relationship-building skills and high-level documentation accuracy. The role involves guiding residents through a critical recovery and housing transition period while ensuring all services are properly documented in the Electronic Health Record (EHR) system. The ideal candidate thrives in structured environments, stays organized, and takes pride in accurate documentation. Residents in this program are medically stable but not ready for independent living, recovering from hospitalization, illness, or injury, and often face housing instability. The role ensures residents' time is used intentionally and documented accurately.

Requirements

  • Detail-oriented and disciplined with daily follow-through.

  • Organized and able to manage multiple resident files.
  • Clear and professional in written communication.
  • Able to balance relationship-building with structure and compliance.
  • Experience working in an EHR or documentation system.

Responsibilities

  • Conduct consistent resident check-ins, assess needs, goals, and recovery progress.
  • Support stabilization after hospitalization and coordinate housing-related activities.
  • Assist with discharge planning and transitions.
  • Communicate with house staff and external partners.
  • Complete daily housing notes for 100% of active residents, ensuring documentation reflects engagement and services provided, and aligns with authorized services and approved length of stay.
  • Maintain organized, audit-ready resident charts and prevent documentation backlogs.
  • Support housing searches and application processes when authorized for Housing Navigation services.
  • Coordinate with landlords, housing providers, and community resources.
  • Track housing search efforts, applications, and placement progress.
  • Complete required monthly Housing Navigation documentation and document outcomes clearly and consistently.
  • Monitor resident length of stay and support timely and appropriate discharges.
  • Respond quickly to documentation feedback and follow established workflows and templates.
  • Maintain charts in review-ready condition at all times.
