About The Position

Central Vermont Home Health & Hospice is seeking a Full-Time Registered Nurse to deliver goal-oriented, individualized home healthcare to community members across central Vermont. Operating as a Case Manager when serving as the lead discipline, this role applies the nursing process under physician orders to support holistic health promotion, long-term care, and maternal-and-child health services across all stages of life.

Requirements

• Active Registered Nurse (RN) license in good standing.
• Minimum 1 year of clinical experience in Home Health setting with OASIS proficiency.
• Direct Home Health experience required.
• Hands-on OASIS documentation experience is strictly required.
• COVID-19 Vaccination Required (Strict policy: No exemptions).

Nice To Haves

• Prior home health travel nurse experience with strong case management skills.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Provide direct, individualized nursing care utilizing the nursing process in accordance with provider orders.
• Act as Case Manager for primary cases, overseeing care planning, patient outcome tracking, and interdisciplinary coordination.
• Complete accurate clinical assessments and documentation utilizing the OASIS tool.
• Educate patients and family members on health management, medication regimens, and self-care strategies.
