About The Position

The Mental Health Care Manager function is to act as a coordinator in providing appropriate care management services to no more than 20 clients diagnosed with a Serious Mental health condition, and identified as a Health Home Plus (HH+) client in accordance with the requirements outlined by NYSDOH, HH and OMH.

Requirements

  • A Master’s degree in one of the qualifying 6 fields and one (1) year of Experience; OR
  • A Bachelor’s degree in one of the qualifying fields and two (2) years of Experience; OR
  • A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and two (2) years of Experience; OR
  • A Bachelor’s degree or higher in ANY field with either: three (3) years of Experience, or two (2) years of experience as a Health Home care manager serving the SMI population.
  • Experience must consist of: a. Providing direct services to people with Serious Mental Illness, developmental disabilities, alcoholism or substance abuse, OR b. Linking individuals with Serious Mental Illness, and/or alcoholism or substance abuse to a broad range of services essential to successful living in a community setting (e.g. medical, psychiatric, social, educational, legal, housing and financial services).
  • Meets physical and cognitive requirements needed to complete assigned tasks and duties of the position with reasonable accommodations being made for individuals with disabilities
  • Ability to travel within the five boroughs of New York City via public transportation
  • Ability to demonstrate proper judgment and decision making skills when necessary
  • Excellent written, verbal, and organizational skills
  • Ability to manage time effectively
  • Knowledge of computers

Responsibilities

  • Be familiar with Program Manual (with particular attention to the specific functions of the Care Manager’s position). This manual was created in accordance with procedures based upon AIDS institute NYSDOH and Health Home guidelines.
  • Complete an initial assessment; subsequently completing and overseeing a client-focused care plan, detailed and mutually developed with each participant.
  • Advocacy and knowledge of available and appropriate services for participants
  • Conduct home visits as required and as needed (minimum of 2 home visits per month per client)
  • Have at least 2 “other” types of contact with each client monthly (phone, email, text etc)
  • Conduct regular case conferences with outside providers on an as-needed basis (NOTE: Care Manager must ensure that at least one case conference is conducted at least once per service plan period for every client).
  • Being available for crisis intervention, having to remain with a client until the issue is resolved or addressed.
  • Complete accurate and up to date documentation and consents for services. Conduct daily follow ups to address any issues, review the previous day's activities, and to discuss any client issues and/or new insights regarding a particular client
  • Be knowledgeable and up-to-date in areas pertaining to serious mental illness (SMI), symptoms and medications, also other chronic illnesses.
  • Be able to work in and create a positive environment, manage providers, delegate and oversee tasks/duties, and address conflicts as they arise.
  • Communicate effectively with supervisory personnel in a cooperative and supportive manner.
  • Maintain monthly per member per month billing standards, meeting one of the 5 core Health Home functions four times monthly for each client
  • Complete the New York State Eligibility Assessment for clients who are deemed eligible for HCBS services for HARP clients. If a client is found eligible for HCBS services, the Care Manager must complete the New York State Community Mental Health Assessment (CMHA).
  • Create an individual-centered plan of care that integrates behavioral and physical health services and includes HCBS for clients who are found eligible.
  • Coordinate with a client’s Managed Care Organization (MCO’s) in order to ensure a client receives the HCBS service(s) they are eligible for.
