About The Position

Parx Home Health Care is seeking an inspiring Clinical Manager to guide clinicians in delivering safe, effective, patient-centered home health care. This role helps ensure patients receive high-quality service and field staff have the tools, guidance, education, resources, and support needed to care for patients appropriately. The Clinical Manager works with agency leadership to uphold standards and continually improve clinical performance, while fostering a culture of integrity, compassion, and excellence.

Requirements

• Licensed RN.
• At least 3 years of home health experience.
• At least 1 year of leadership experience.
• Ability to use OASIS, EMR platforms, and clinical documentation systems.
• Ability to lead teams and help team members feel valued.
• Organized, compassionate, and able to remain calm under pressure.

Responsibilities

• Lead and support a team of clinicians to ensure quality, compliance, and continuity of care.
• Oversee coordination of patient services, care planning, and interdisciplinary communication.
• Collaborate with physicians, patients, and families to personalize care.
• Ensure field staff receive the education, resources, and real-time support needed to deliver care.
• Partner with leadership to uphold agency standards and improve clinical performance.
• Foster a culture grounded in integrity, compassion, and excellence.
• Mentor, coach, and train new team members.
