Under direct and indirect supervision, provides care management functions, documentation, referral and linkage, and monitoring/follow-up. The Care Management Extender works in conjunction with the client, family, friends, and providers who have lengthy experience with the person to assist them in obtaining desired outcomes/skills/symptom reduction. This role facilitates the provider choice process, maintains objectivity, and provides fact-finding assistance. Ensures that signed Authorization to Disclose Health Information forms are obtained and on file when needed, and that all information released/disclosed is documented. Referral and linkage activities connect recipients with medical, behavioral, social, and other programs, services, and supports to address identified needs and achieve goals specified in the Care Management Plan. This includes coordinating services, facilitating access, making referrals, scheduling appointments, assisting with transitions between levels of care, facilitating communication among providers and the recipient, and assisting with establishing medical homes, OBGYN, and prenatal care as necessary. The extender also assists consumers in considering and accessing natural community supports and ensures paid supports are coordinated with other community resources. Monitoring and follow-up activities ensure the Care Management Plan is effectively implemented and addresses recipient needs, determining if services are provided as planned, are adequate and effective, if there are changes in recipient needs, and if the recipient is progressing toward goals. Documentation of monitoring and actions taken is required. The extender ensures monitoring schedules are sufficient for health, safety, and welfare, and monitors progress through observation, interview, and documentation review. Coordination involves working closely with consumers, providers, and others to avoid/resolve conflicts and problems hindering effective treatment. Assists consumers in obtaining entitlement services and monitors continued eligibility for Medicaid and/or NC Health Choice.
