About The Position

The Care Manager/Educator is responsible for recruiting and leading the nursing care team in providing care and services for Residents at Meadow Ridge Seniors Village. This role involves liaising with Residents' families, physicians, and Health Authority representatives, as well as coordinating and supervising the nursing care team. The Care Manager provides guidance and direction to ensure Resident-centered care based on individualized needs and care plans, demonstrating knowledge of leadership and gerontology best practices. A commitment to Resident safety and a "culture of safety" is paramount.

Requirements

  • Licensed Practical Nurse with active registration.
  • Three (3) years related experience in a Supportive Living and/or Long-Term Care setting.
  • Communicates fluently in English both verbally and in writing.
  • Demonstrates emotional maturity and ability to motivate employees.
  • Applies problem-solving process demonstrating critical thinking and decision-making skills with systems analysis.
  • Ability to problem-solve clinical issues through knowledge and experience in gerontological, evidence-based practice.
  • Demonstrates ability to work together with others toward shared goals and desired outcomes.
  • Demonstrates ability to effectively establish professional working relationships with Residents, families, physicians, and the Care Team.
  • Demonstrates ability to effectively introduce and manage purposeful change.
  • Demonstrates ability to communicate effectively with Residents, families, the public, medical staff, and the interdisciplinary team.
  • Ability to intervene effectively to resolve conflict and manage stress and work pressure.
  • Ability to apply continuous quality improvement tools and processes.
  • Demonstrates applicable computer skills and knowledge of nursing-related care equipment utilization and management.
  • Responsible to read, understand and comply with Park Place and provincially mandated Occupational Health and Safety policies and safe work practices.

Responsibilities

  • Recruits, orients, leads, and supervises the nursing care team to ensure effective, consistent, and comprehensive Resident care planning and delivery.
  • Ensures holistic, comprehensive assessments of Residents and families are completed and used as the basis for individualized care planning and delivery, including evaluation and revision of care plans.
  • Provides leadership through a respectful, collaborative approach to monitor, supervise, and direct the care team. Offers new nursing care employee orientation, performance appraisals, monthly and mandatory education, mentoring, and monitors employee performance, which may lead to discipline.
  • Acts as a role model by demonstrating leadership, professionalism, empathy, trust, and respect in all interpersonal relationships.
  • Establishes and maintains therapeutic relationships with Residents and families, employees, volunteers, and others.
  • Supports wellness staff in advocating for Resident choice and autonomy in decision-making and care planning, offering ongoing education and advocacy.
  • Ensures applicable staffing levels and monitors nursing team collaboration and processes to ensure quality Resident-centered care.
  • Leads quality improvement/performance measurement initiatives and processes in collaboration with the Site Leader or Director of Care, including follow-up and action plan implementation.
  • Focuses on quality of care and safety assurance and improvement by taking a leadership role in site committees.
  • Ensures all documentation of assessments, records of observation, care provision, and Resident responses are according to professional standards and policies.
  • Leads the nursing care team in planning, organizing, establishing priorities, using resources effectively, responding to unanticipated events, reassigning tasks, and ensuring staff are aware of assignments.
  • Leads the nursing care team in focusing on safety by exhibiting accident prevention, knowledge of emergency procedures, safe use of resources and equipment, and effective infection prevention and control.
  • Presents and/or attends in-service and other educational programs as required.
  • Performs other related duties as assigned.
  • Responsible to educate employees annually on ‘Violence in the Workplace” and “Bullying and Harassment” in the workplace.
  • Demonstrates leadership through involvement in all aspects of the Health and Safety Program, including promoting, instructing, and monitoring safe work procedures.
  • Responsible to take every reasonable precaution to protect the safety of Residents, self, other employees, and the public.
  • Reports any near miss, injury, accident, or equipment damage to supervisor immediately and completes required reports.
  • Corrects and/or reports unsafe conditions.
  • Leads by example in health and safety.
