About The Position

We are seeking a Clinical Scheduler to fill our Care Manager role assist with all clinical scheduling needs for our Marietta patients. The role serves as the central scheduling and operational coordinator for a single interdisciplinary care team, ensuring that patients receive the appropriate number and type of in-person encounters required under the Your Health proactive, value-based care model. This position will be working in the Marietta office daily. This is a full-time, salary-based 8-hr position (8:00AM-5:00PM) (Monday-Friday).

Requirements

  • High school diploma or equivalent required
  • Previous experience in a medical or healthcare setting, preferably a scheduling or administrative role.
  • Familiar with medical terminology, procedures, and insurance verification purposes.
  • Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
  • Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization.
  • Strong problem-solving abilities
  • Ability to work effectively under pressure.

Nice To Haves

  • Associate's or Bachelor's Degree in Healthcare is preferred.
  • Preferred CMA, LPN or RN

Responsibilities

  • Coordinating all in-person facilitator visits and in-person provider encounters for patients assigned to their care team.
  • Ensuring patients receive consistent monitoring, engagement, and care team oversight through effective scheduling and coordination.
  • Translating clinical recommendations into a clear and organized patient visit schedule in collaboration with Directors of Population Health (DPH), providers, facilitators, and specialty population health divisions.
  • Appropriately registering, verifying, and timely scheduling patients according to the Company’s care model.
  • Ensuring new patients are being tracked and followed until scheduled.
  • Scheduling appointments for patients according to the care model, including follow-ups with healthcare providers based on availability, medical urgency, and patient preferences.
  • Ensuring all disciplines with schedules are fully optimized.
  • Ensuring patients are being followed and scheduled appropriately when discharged from an in-patient facility.
  • Ensuring TCM appointments are scheduled promptly and within the required window.
  • Implementing strong documentation.
  • Utilizing a multitude of software systems (including tracking systems) to ensure patient follow-up and better patient outcomes.
  • Interacting with patients via email, patient portal, telephone, AthenaText, in-person, etc. to gather necessary information, schedule and confirm appointments, and provide pre-appointment instructions.
