About The Position

Humana Healthy Horizons in Virginia is looking for a Care Management Support Assistant to support the administration of care management. This virtual team performs moderately complex administrative support tasks for the Commonwealth of Virginia Medicaid program. The role contributes to the administration of care management by managing shared mailboxes, entering data into work queues, mailing letters/educational material, researching unable to contact members, and completing other administrative functions as assigned. The position ensures the timely and correct submission of Commonwealth-required deliverables, researches alternative contact information for hard-to-search members, contacts members to engage them in care management programs, and supports inbound call escalations. This role provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state. The assistant interacts with Coordinators (Care Coaches) and Field Care Managers who guide members and their families toward and facilitate interaction with resources appropriate for care and well-being. The role performs varied clerical activities including reporting data, tracking data, pulling reports, and monitoring/managing email. The position involves performing computations and typically works on semi-routine assignments that are subject to change. The assistant may make, receive, and soft transfer calls to and from members, providers, and family, ensuring timely communication and follow-up occurs with care management.

Requirements

  • Candidate must reside in Virginia
  • Less than 3 years of technical experience with Microsoft Word, Excel, Outlook
  • Administration support experience in a healthcare industry
  • Familiarity with care and well-being resources
