About The Position

The Care Management Supervisor works under the supervision of the Care Management Director. This position performs all essential functions of the Community Care Manager, including care management, care coordination, group facilitation, and direct service delivery, while providing day-to-day leadership, administrative support, and program coordination for the Care Management team. The Care Management Supervisor serves as a resource and team lead to support program operations, regulatory compliance, staff development, quality improvement, and program growth. This position maintains a direct client care assignment while assisting with specialized programs, contracts, training initiatives, and other administrative responsibilities as assigned.

Requirements

  • Master’s Degree in a human services related field OR Bachelor’s degree in a human services related field and 1 year relevant experience working with the target population preferred OR High School Diploma or GED with a minimum of 3 years of experience working with the target population OR Certified Peer Specialist (CPS) or Certified Family Specialist (CFS)
  • A minimum of two years working with target population.
  • One year demonstrated supervisory experience preferred.
  • Valid drivers’ license and meets the driving privileges guidelines established in HR Policy 12.6 “Vehicle Safety and Client Transportation”
  • Ability to demonstrate all functions of clinical program staff.
  • Ability to manage time to meet all deadlines.
  • Ability to recognize and identify symptoms of mental illness and suicide and risk potential and respond appropriately.
  • Good verbal and written skills.
  • Valid driver’s license
  • CPR/First Aid
  • CPI

Responsibilities

  • Performs all essential functions of the Community Care Manager position, including care management, care coordination, direct service delivery, and facilitation of therapeutic and psychoeducational groups as assigned.
  • Provides individual interventions to develop or enhance clients' ability to make informed, independent choices, improve functioning, and achieve person-centered goals.
  • Coordinates services across behavioral health, physical health, community providers, and natural supports to promote integrated, person-centered care and positive client outcomes.
  • Participates as a member of the multidisciplinary treatment team in the development and implementation of person-centered care plans, safety plans, and relapse prevention plans as indicated.
  • Demonstrates the ability to recognize and assess suicide risk, behavioral health symptoms, substance use concerns, physical health needs, and other crisis situations, responding appropriately within agency standards.
  • Provides comprehensive outreach, engagement, care coordination, and follow-up services for assigned individuals.
  • Provides in on-call and crisis response/support responsibility as assigned.
  • Demonstrates the ability to engage and develop rapport with individuals presenting with a variety of behavioral health, substance use, and medical needs.
  • Provides services that are person-centered, culturally responsive, non-judgmental, and welcoming to diverse populations.
  • Serves as a team lead by providing day-to-day guidance, mentoring, support, and clinical consultation to Care Management staff.
  • Provides consultation and support to Care Management staff regarding care coordination, service planning, community resources, documentation, engagement strategies, and complex client needs.
  • Assists leadership in coordinating daily program operations to promote quality service delivery, efficient workflow, appropriate staffing coverage, and program growth.
  • Supports program compliance with agency policies, contractual requirements, accreditation standards, and state and federal regulations.
  • Assists with program monitoring activities including documentation review, quality improvement initiatives, audits, productivity tracking, referral management, census management, and data collection.
  • Assists with onboarding, orientation, training, coaching, and ongoing professional development of Care Management staff.
  • Supports specialized programs, grants, contracts, and agency initiatives within Care Management as assigned.
  • Collaborates with multidisciplinary teams, internal departments, community partners, referral sources, managed care organizations, and other stakeholders to promote effective care coordination and positive program outcomes.
  • Demonstrates excellent organizational, communication, leadership, problem-solving, and time management skills while balancing direct client care with administrative responsibilities.
  • Documents all services within expected timeframes in accordance with service definitions, regulatory standards, quality improvement goals, and agency expectations.
  • Consistently meets established productivity, client care, documentation, and program performance expectations.
  • Participates in and completes all required training, supervision, and professional development activities in an accurate and timely manner.
  • Completes other administrative support duties, leadership responsibilities, and special projects as developed and assigned.
