About The Position

The Treatment Coordinator will work under the supervision of the Program Manager and Clinical Director/Supervisor. This position will focus on provision of person-centered case manager and coordination service to children in the NMS TFC program.

Requirements

• Master’s Degree in the following fields: social work, clinical psychology, family studies, or another human services related field. OR Bachelor’s Degree in the following fields: social work, clinical psychology, family studies, or another human services related field.
• Two years experience working in the social services field preferred. Master’s degree in applicable field listed above can substitute for two years experience credit.
• Background in child mental health field preferred.
• A bachelor’s degree in a non-human services field plus three years’ experience working with children, youth, and families may be a substitute for a Bachelor’s degree in a Human Services Related field plus two years experience.
• Valid drivers’ license and meets the driving privileges guidelines established in HR Policy 12.6 “Vehicle Safety and Client Transportation.”
• Demonstrated success in the following areas: Ability to manage time to meet all deadlines.
• Ability to recognize and identify symptoms of mental illness and suicide and risk potential and respond appropriately.
• Good verbal and written skills.

Nice To Haves

• Bilingual (English/Spanish) is preferred but not required.

Responsibilities

• Participates as an active treatment team member serving children enrolled in NMS TFC program as evidenced by: Meeting all State requirements regarding contact for all treatment foster children including a minimum of two face-to-face contacts per month (as defined by supervisor). The ability to use both in-home and out of home contacts for the purpose of hearing any special concerns the child may have. Contacts are used to assess the child’s progress and identify the child’s treatment needs, and monitor any potential neglect/abuse.
• Scheduling and completing treatment team meetings in a manner that fosters team cooperation and meets agency and state requirements.
• Demonstrating the ability to develop and implement each child’s treatment plan in coordination with the treatment team: (1) prepares the initial written treatment plan with the treatment team, (2) supervises the implementation of the treatment plan, (3) reviews and monitors the treatment plan, (4) develops and documents modifications in the treatment plan, and 5) assures that appropriate signatures are obtained from appropriate treatment team members.
• Demonstrating the ability to complete all case management and coordination type of needs for each assigned treatment foster child and each treatment foster parent. This includes but is not limited to providing support to each treatment foster parent to whom they are assigned.
• Demonstrating the ability to follow up on all goals from treatment team, as well as, recommendations made by the supervisor in and out of formal supervision.
• Demonstrating support of the child’s relationship with his/her biological family and involve them in their child’s treatment planning (as appropriate).
• Participating in scheduled meetings, committees, supervision sessions, and team meetings.
• Participating in on-call rotation. Providing appropriate support and seeking out clinical supervision as appropriate when on-call.
• Participates as an active treatment team member serving individuals in the TFC program as evidenced by: Demonstrating the ability to recognize and identify symptoms of mental illness and substance use.
• Participating as a member of a multi-disciplinary team approach to treatment and recovery.
• Coordinating with both internal and external providers regarding service provision.
• Providing training and support to new TCs as assigned.
• Provides comprehensive outreach, engagement, and follow up to assigned individuals as evidenced by: Providing comprehensive outreach and follow up to all assigned individuals including contact attempts over the phone, in-person, in the home etc.
• Demonstrating the ability to engage and develop rapport with individuals presenting with a variety of symptoms/presentations.
• Provides culturally appropriate services to diverse populations as evidenced by: Providing services that are person-centered.
• Providing services that are culturally relevant.
• Providing a non-judgmental, open, and welcoming approach to diverse populations.
• Completed documentation that meets identified expectations as evidenced by: Documenting all services provided within expected time frames.
• Documenting all services provided in a manner that meets service definitions, regulatory standards, and agency expectations.
• Documenting all services provided in a manner that is person-centered and meets quality improvement goals.
• Consistently meeting established productivity standards as evidenced by: Demonstrating excellent organizational skills.

• Participates in staff development activities to increase skills and professional development as evidenced by: Participating in and completing all required trainings in an accurate and timely manner including foster care specific training of 40 hours.

• Attending and participating in supervision sessions and demonstrating the ability to apply supervisory recommendations to work performance.
• Completes other administrative support duties and special projects as developed and assigned.
