About The Position

Mental Health Cooperative, Inc. (MHC) is seeking a Care Management Supervisor for Homeless Services in Nashville, TN. MHC is dedicated to supporting and treating individuals challenged with serious mental illness and poverty. This role provides leadership, supervision, and support to a team delivering services to individuals experiencing homelessness, involved in the court system, and those with severe and persistent mental illness. The schedule is Monday to Friday, 8:00 AM-4:30 PM, with a base salary of $53,500 and potential incentives. Mileage reimbursement is available.

Requirements

  • Valid Tennessee Driver’s License
  • Acceptable Motor Vehicle Report (MVR)
  • Acceptable Criminal Background Investigation
  • Personal Automobile Insurance
  • Two years of supervisory experience in a mental health setting.

  • Master’s degree (M.A., M.S.) in a health-related field of counseling, psychology, or social work
  • Licensed or license eligible

Responsibilities

  • Provide leadership, supervision, and support to a team of assigned staff serving individuals experiencing homelessness, involved in the court system, and consumers with severe and persistent mental illness.
  • Supervise care management and outreach teams, ensuring consumers are seen according to agency standards and grant requirements.
  • Provide appropriate clinical and administrative recommendations.
  • Ensure program protocols, policies, and procedures are carried out.
  • Conduct planning sessions and meetings with the team as required by the Agency and as needed.
  • Coordinate prompt assignments of consumers to care managers and ensure appropriate contact.
  • Promptly address team concerns and work to obtain resolution.
  • Provide on-site consulting with the Office Coordinator, Program Manager, and Care Managers as needed or assigned.
  • Offer support and consultation for care managers with consumers in crisis situations.
  • Ensure consumers are seen upon admission and discharge from the hospital and respite within Agency and BHO standards.
  • Ensure consumer’s daily psychiatric, medical, and environmental needs are planned for and coordinated.
  • Review care management documentation to ensure it is individualized, meets consumer needs, and shows medical necessity for continued service.
  • Ensure consumers are provided efficient transitions within the Agency and into the community.
  • Assist care managers with referral and linkage to community resources and advocate for consumers when necessary.
  • Meet regularly with the Program Manager for supervision (individual and/or group).
  • Attend regular Supervisor’s meetings.
  • Attend and facilitate daily and weekly team meetings, ensuring a multi-disciplinary approach and appropriate documentation.
  • Counsel care managers on performance and complete scheduled performance appraisals.
  • Collect and monitor completion of clinical documentation as outlined in Agency policy and procedures (e.g., hospital, 90-day housing review).
  • Collect and review all appropriate documentation for payroll and financial reimbursement and submit to the Accounting Department in a timely manner.
  • Interpret necessary agency reports and use them to monitor production standards of assigned care managers/outreach specialists as outlined in the Agency policy and procedure manual and per grant requirements.
  • Provide individual supervision of assigned Care Managers and Outreach Specialists, maintaining supervision records in accordance with Agency and BHO standards. Supervision includes in-office meetings, ride-alongs, and chart audits.
  • Assist in the hiring of care management staff.
  • Ensure timely and appropriate documentation as required by the Agency or grantee, including the use of the HMIS database.
  • Develop and maintain community relations through prompt follow-up with contacts.
  • Participate in community-coordinated events and initiatives put in place by the Metro Nashville Continuum of Care, including community meetings required or necessary for grant participation.
  • Assist Program Manager in the completion of quarterly and annual reports for PATH Program and agency reports.
  • Attend and assist in site monitoring visits.
