About The Position

Fast-paced, high-volume clinic looking for a top skilled Care Management Specialist. This role is full-time, 40 hours weekly, Monday through Friday, from 8:00 am to 5:00 pm. Requirements

  • High School Diploma or equivalent (REQUIRED)
  • Four years of work experience in an Outpatient Healthcare Facility (REQUIRED)

Nice To Haves

  • Healthcare Training Certification (PREFERRED)
  • Customer Service work experience in Insurance or healthcare (PREFERRED)
  • CMA (STRONGLY PREFERRED)
  • Community Health Worker Certification (PREFERRED)

Responsibilities

  • Educates and guides patients to understand the benefits and requirements of care management services as needed.
  • Receives Chronic Care Management and Remote Physiological Monitoring orders from Neurology Physicians, accesses patient charts and imports information into Care Management dashboard systems (Healtharc, Prevounce, Allscripts).
  • Engage patients by phone, text, or video to: Remind patients to record their vitals; respond to alerts related to patient symptoms or physiologic data; respond to patients for minor technical troubleshooting.
  • Works with patients on determining insurance eligibility and authorization requirements for care management support.
  • Assists patients with coordination of appointments and other care management needs related to their chronic conditions.
