About The Position

Carolina Outreach is seeking a Care Management Extender to join their Tailored Care Management team. This role supports individuals with behavioral health needs by coordinating care, linking them to services, and monitoring their progress. The ideal candidate is highly organized, possesses excellent time management skills, and can manage a high volume of calls while building rapport with individuals who have complex needs. This position requires experience in a performance-driven environment with established productivity standards and quality metrics. The Care Management Extender plays a crucial role in integrated care by supporting members through collaborative efforts to improve outcomes, including medication management, coordinating physical and behavioral health care, educating members, building trust, supporting community recovery programs, and enhancing communication and continuity of care.

Requirements

A high school diploma or equivalent
Must be 18 years or older
Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist or A parent or guardian of an individual with a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual or Have two years of paid experience performing the types of functions described in the “Extender Functions” section, with at least one year of paid experience working directly with the Tailored Care Management eligible population.
Upon hire, completion of general onboarding training
Within 60 days of hire, completion of NC DHHS identified training requirements for Tailored Care Management
No driving infractions in the last 5 years
Must be authorized to work in the United States.

Responsibilities

Conduct regular outreach to members by phone and in person to assess needs, provide support, and monitor progress.
Connect members and families to healthcare services, housing assistance, food resources, and community programs.
Assist in developing and updating individualized care plans to support members' health and wellness goals.
Coordinate communication among healthcare providers, members, and families to ensure continuity of care and follow-up on referrals.
Monitor members (in person and by telephone) for changes in clinical symptoms and treatment side effects or complications.
Evaluate findings and assist members/families in developing an Individualized Service Plan (ISP) with appropriate goals.
Support and closely coordinate whole-person health care with the member’s primary care provider and other treating health providers.
Provide or facilitate resources internally or in the community for members/families, and make referrals and provide follow-up as appropriate (e.g., social services such as housing assistance, vocational rehabilitation, substance use treatment).
Ensure all treating providers involved in the member’s care are up to date on the latest test results and information.
