Care Management Coordinator – RN – Ambulatory - Days
About The Position
The Ambulatory Care Network (ACN) is part of the Division of Community and Population Health of New York-Presbyterian, one of the nation’s most comprehensive academic health care delivery systems affiliated with two world-renowned medical schools. Through primary care and multispecialty medical practices, the physicians and other health care professionals of the ACN, in collaboration with Columbia Doctors and Weill Cornell Physicians, provide families with high quality, patient-centered care close to home in the New York metropolitan area. This role is responsible for providing care management to high risk patients in the local community to improve the management of their chronic illness and to decrease unnecessary emergency and inpatient admissions. Conduct case-finding, assessments, education, counseling and other interventions to patients who are targeted through chronic disease registries. Assist patients in navigating NYP service delivery system and other services to overcome barriers to self-management and adherence. Connect patients and their families to community resources and ensure the proper continuation of quality care.
Requirements
- BSN (Required)
- Current NYS RN Licensure (Required)
- 5+ years of healthcare experience (Required)
- Experience with electronic medical records (Required)
- Computer proficiency in Word and Excel (Required)
- Excellent interpersonal skills (Required)
- Ability to problem solve and prioritize (Required)
- Flexibility and ability to work in fast-paced, changing environment (Required)
Nice To Haves
- Masters in Nursing or related field. If Masters in Nursing, BSN can be preferred
- PRI Certification (Preferred)
- 2+ years of experience in case management in an inpatient, HMO, Insurance, or other outpatient setting (Preferred)
- Bilingual English and Spanish (Preferred)
