About The Position

This role is responsible for providing care management to high-risk patients in the local community to improve the management of their chronic illness and to decrease unnecessary emergency and inpatient admissions. The coordinator will conduct case-finding, assessments, education, counseling, and other interventions for patients targeted through chronic disease registries. They will assist patients in navigating the NYP service delivery system and other services to overcome barriers to self-management and adherence, and connect patients and their families to community resources to ensure the proper continuation of quality care.

Requirements

BSN
Current NYS RN Licensure
5+ years of healthcare experience
Experience with electronic medical records
Computer proficiency in Word and Excel
Excellent interpersonal skills
Ability to problem solve and prioritize
Flexibility and ability to work in fast-paced, changing environment

Nice To Haves

Masters in Nursing or related field. If Masters in Nursing, BSN can be preferred
PRI Certification
2+ years of experience in case management in an inpatient, HMO, Insurance, or other outpatient setting
Bilingual English and Spanish
