Care Integration Coordinator Full- time Day
About The Position
The Care Integration Coordinator at Providence St. Joseph Hospital in Eureka, CA, is a Full-time position working 8-hour Day shifts. This role is part of a statewide initiative to improve access to Medication for Opioid Use Disorder (MOUD). The MOUD Navigator acts as a knowledgeable resource for Substance Use Disorder (SUD) and MOUD treatment programs, bridging the gap between the Emergency Department and various treatment settings. The navigator connects patients to resources for care management, advocates for MOUD services using a Trauma Informed approach, and collaborates with community leaders, physicians, and the healthcare team to advocate for MOUD services. Requirements
- Associate's degree, or equivalent education/experience
- Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving.
- Caregivers will be expected to provide proof of driver license and auto insurance upon request.
- 1 year of experience in community social services, case management, or similar setting.
Nice To Haves
- Bachelor's Degree
- Upon hire: Certified Addictions and Treatment Counselor.
- Substance Use Program or medical setting experience.
Responsibilities
- Serve as a consistent and knowledgeable resource of Substance Use Disorder (SUD) and Medication for Opioid Use Disorder (MOUD) treatment programs.
- Serve as a bridge between the Emergency Department and treatment settings including outpatient care, inpatient care, MOUD/SUD treatment programs.
- Connect patients to resources for management of care needs.
- Serve as an advocate to patients in the emergency department who may be interested in MOUD services and support, utilizing a Trauma Informed approach.
- Collaborate with community resource leaders, physicians, and other members of the health care team.
- Cultivate relationships and serve as a community advocate for the needs of MOUD services.
