About The Position

Wintringham’s Williamstown Hostel is seeking an experienced Care Evaluator to join their team. This role involves providing clinical guidance, care planning, and hands-on support to residents, working alongside residents and care staff to promote independence, dignity, and quality of life. The Care Evaluator will deliver high-quality, person-centred care, assess residents' needs, develop and update Support Plans, build a strong understanding of individual care needs, monitor changes in residents' health and behaviour, and provide clinical guidance to care staff.

Requirements

Experience in gerontology / community / palliative / mental health / alcohol and drugs or related specialty
Registered Nurse Division 1
Well-developed assessment and documentation skills
Knowledge and understanding of the special needs of older people
Ability to liaise with staff, residents, their families and other professionals
Australian Citizen, Permanent Resident or hold a valid work permit or visa.

Responsibilities

Deliver high-quality, person-centred care that promotes residents’ independence, dignity and lifestyle choices.
Assess residents’ needs and develop, review and update Support Plans in partnership with residents and their care teams.
Build a strong understanding of each resident’s care needs through hands-on care and close collaboration with care staff.
Monitor changes in residents’ health, medication, behaviour and lifestyle, developing appropriate strategies to respond to changing needs.
Provide clinical guidance and support to care staff, working collaboratively to maintain high standards of care and achieve positive outcomes for residents.
