About The Position

The Care Coordinator works within the Soft Landing Program at Reading Hospital. This program provides support to pregnant and postpartum women struggling with substance use, serving those who are currently using substances, those in recovery, and individuals utilizing Medication-Assisted Treatment. The program offers patients care coordination, therapy, medical care referrals, recovery and mental health resources, pregnancy and parenting support, and assistance with applying for resources such as food assistance and housing.

Requirements

Thorough knowledge of substance use disorder, case management theory, motivational interviewing, child development and medication assisted treatment as well other available treatment options and maternal health needs.
