The Care Coordinator acts as the primary consumer advocate for the assigned program. This role is responsible for staying informed about consumer progress and coordinating all services delivered to the consumer and their family, both internally and through external referrals. The position involves administering assessments, developing and reviewing service plans, documenting clinical services, conducting case management sessions, providing vocational education, attending external appointments on behalf of consumers, making referrals, preparing progress reports, coordinating care with treatment teams and external providers, developing discharge plans, arranging transportation, and participating in case conferences. The role also requires understanding HIPAA, professional ethics, consumer rights, and confidentiality, as well as completing all required training and embracing the mission and values of WestCare Foundation.
