About The Position
The Rural Pathways program is a new SAMHSA-funded initiative designed to expand access to evidence-based treatment, medication for opioid use disorder (MOUD), recovery support services, and behavioral healthcare across northeastern Connecticut. The program utilizes innovative service delivery models, including CHR's Mobile Pathways Unit, outpatient clinics, and community partnerships, to reduce barriers related to transportation, geography, and provider shortages. Rural Pathways focuses on rapid access to care, overdose prevention, integrated treatment for co-occurring mental health and substance use disorders, and long-term recovery support for individuals living in rural communities. The Care Coordinator will participate as a member of a multi-disciplinary treatment team, track and coordinate client intake, discharge, services, and referrals. They will provide assistance to the team, including coordination of treatment planning and providing daily triage. The role involves educating and linking individuals to self-help groups, natural support, and other resources, as well as assisting with linking clients to community support systems and agencies to obtain benefits such as insurance, housing, treatment, representative payee, legal services, medical services, education, and employment. Maintaining careful documentation of contacts and communication is crucial for quality services and compliance with agency policy. The Care Coordinator will act as a liaison between the agency and other relevant providers and teams to coordinate care, and may assist with scheduling and providing transportation to and from appointments. The position requires participation in staff meetings, supervision, training, and other assigned meetings. Establishing knowledge of cultures and communication styles associated with various groups, particularly oppressed groups, is essential for providing culturally sensitive and competent care. Crisis intervention and support will be provided as clinically indicated. Additional duties may be required.
Requirements
- Bachelor’s degree in human service-related field required.
- A minimum of two years’ related experience preferred.
- Valid driver’s license required.
- Complete any program specific training required.
- Complete required annual agency trainings.
- CPR/First-Aid Certification may be required within 6 months of hire
