OTP Care Coordinator CODA, Inc. | Opioid Treatment Program (OTP) Full-Time | Exempt | Monday–Friday (Day Shift) Be the Connection That Keeps Patients Engaged in Recovery. Requirements

  • Bachelor’s Degree in Social Work or related field (preferred)
  • 2+ years experience in care coordination, healthcare, or social services
  • Strong understanding of community resources and patient engagement strategies
  • Ability to analyze data and translate it into action
  • Strong communication and relationship-building skills
  • CADC I (or ability to obtain within 24 months; CADC-R within 30 days minimum
  • CPR Certification (within 90 days)
  • Crisis Prevention Training (within 90 days)
  • Valid OR/WA Driver’s License (if required for role)
