This position serves as the facilitator of child and family teams, advocating for families, providing linkage with needed community resources, assisting with treatment and education, and offering crisis support and safety planning. The role involves adhering to agency policies, CARF and DMHSAS standards, confidentiality rules, and funding source requirements. The Care Coordinator facilitates the wraparound process, ensuring teams move families toward stability, and encourages active participation from all team members. They partner with professional and natural supports, respect confidentiality, and conduct strengths, needs, and crisis assessments. A key responsibility is developing strength-based, family-guided wraparound plans and coordinating the process to keep children in their homes and schools, avoiding out-of-home placements. The role empowers children, youth, and families through modeling, advocacy, coaching, and support, teaching them to advocate within various child-serving agencies. The Care Coordinator also develops community relationships, serves as a liaison, and participates in community education. They monitor eligibility for benefits like Social Security and Medicaid, maintain client information, and assist clients in accessing community resources and developing independent living skills. The position requires strict confidentiality, a minimum of 85 billable service hours per month, and collaborative documentation. The work schedule may include late days to accommodate family schedules, and the ability to provide services in community settings and client transportation is necessary.
