About The Position

Heritage Behavioral Health Center is hiring a Care Coordinator to join our outpatient mental health and substance use teams. This role is crucial for helping people navigate their healthcare journey and has a real impact on client care, while also offering work-life balance. Heritage Behavioral Health Center is a Certified Community Behavioral Health Clinic serving individuals across a multi-county area in and around Decatur, Illinois. They believe every team member plays a part in client care and prioritize taking care of their staff.

Requirements

  • High School Diploma/GED plus 5 years of supervised mental health or social service experience, OR a Bachelor's degree in social work, psychology, education, or a related human services field
  • Minimum age of 21
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • Strong communication skills and the ability to build rapport with diverse clients
  • Comfort with electronic health records (EHR) and Microsoft Office
  • A collaborative, adaptable, and empathetic approach to client care

Responsibilities

  • Ensure clients get connected to behavioral health treatment, primary care, specialty care, and community resources
  • Follow up proactively with clients to track progress and troubleshoot barriers to care
  • Coordinate with primary care teams, behavioral health providers, and community partners on appointments and treatment plans
  • Educate clients and families on care continuity and available resources
  • Document client progress and service engagement in electronic health records
  • Help clients navigate healthcare and social service systems
  • Participate in interdisciplinary team meetings and ongoing trainings
