About The Position

The Care Coordinator will support the Blended Case Management (BCM) program at Children’s Crisis Treatment Center (CCTC). This role involves following through with treatment team recommendations in a fast-paced environment to meet the needs of children ages 2-14 and their families who are presenting with crisis and behavioral health issues in a trauma-informed manner. CCTC is dedicated to addressing the impact of childhood traumatic events and other challenges.

Requirements

  • A minimum of a Bachelor’s degree in Social Work, Social Science or Bachelor of Arts Degree from an accredited educational institution.
  • Ability to provide direct services to children and adolescents with serious emotional disturbances and their families.
  • Experience in community families with multiple problems.

Responsibilities

  • Provide interagency assessment and understanding of child’s medical, family, social, physical and educational history.
  • Develop interagency service planning based on the strengths of the child and family and assist in developing any activities which are necessary to help the child and family in the home and community.
  • Support network building for the child, utilizing a system’s approach to include the child’s family and community.
  • Meet with families within their communities once per month or more and develop aggressive and creative strategies to assist the child and family gain access to resources and required services identified in the treatment or service plan.
  • Identify, link, access, coordinate and monitor needed services to keep the child stable in home, school and community.
  • Implement and follow through with psychological or psychiatric recommendations.
  • Monitor the effectiveness of services to ensure continuity of care in the mental health, behavioral and educational system.
  • Network with other child-service systems.
  • Act as a child and system advocate.
  • Provide services with cultural sensitivity.
  • Maintain open communication with team members and supervisor on caseload status.
  • Able to prioritize multiple tasks and meet paperwork deadlines.
  • Assume additional responsibilities as assigned.
