About The Position

The Care Coordinator for the Diversion Program plays a vital role in coordinating services for justice-involved individuals with behavioral health needs. This position is responsible for engaging participants, developing individualized service plans, and ensuring timely linkage to mental health, substance use, medical, housing, and social support services. The Care Coordinator acts as a liaison between clients, the treatment team, legal entities (e.g., probation, public defenders), and community providers to support compliance with Diversion mandates and promote long-term recovery and stability. Key responsibilities include conducting comprehensive needs assessments, maintaining regular contact with participants, tracking progress toward goals, and ensuring care coordination is aligned with program and legal requirements. The Care Coordinator also participates in court proceedings as needed, advocates for client needs, and monitors program metrics to support quality improvement and outcomes reporting. This role requires strong communication, organizational, and clinical coordination skills, as well as the ability to navigate complex systems of care. The ideal candidate is compassionate, detail-oriented, and committed to improving outcomes for individuals involved in the criminal justice system. The Diversion Support Services program works with individuals who are incarcerated in Middlesex County and released under bail reform to link them to services, including housing, financial assistance, physical health and mental health services. The program works from a peer-focused and strengths-based model to bring consumers from surviving to thriving.

Requirements

  • Bachelor’s degree in social work, psychology, human services, or a related field required
  • Strong documentation, organizational, and time management skills
  • Ability to work collaboratively within an interdisciplinary team and across multiple service systems
  • Proficiency in using electronic health records and Microsoft Office Suite
  • Valid driver’s license and reliable transportation required

Nice To Haves

  • Experience working with justice-involved individuals or in behavioral health care coordination preferred
  • Familiarity with mental health and substance use disorder treatment systems, levels of care, and recovery supports
  • Experience coordinating with criminal justice systems (e.g., probation, courts, defense attorneys) preferred
