Under the supervision of the Program Director, the Care Coordinator is responsible for all aspects of care coordination for ROWI clients. This includes coordinating individualized services such as orientation, transportation, and doctor's appointments, as well as managing schedules. The role ensures that programs and services are delivered at the highest standards, providing clients with the utmost level of care. The Care Coordinator also plays a key role in client supervision, ensuring attendance at designated groups, facilitating assigned groups and activities, and conducting client orientations. Responsibilities extend to ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation, administering Urine Analysis, assisting with facility cleanliness, and coordinating client lunches. The position requires serving as a role model, maintaining client confidentiality, complying with the dress code, completing tasks accurately and timely, and attending staff meetings. Additionally, the role involves maintaining clear boundaries, managing clients when direct supervision is needed, and ensuring all EMR documentation is submitted within 24 hours.
