About The Position

Serves as a member of the Healthcare Home team, providing care coordination for clients. This role involves assisting clients with applications for MoHealthNet and other benefits, scheduling appointments, connecting them with community resources, participating in community events, and tracking client data. The Care Coordinator will also educate consumers about MoHealthNet benefits and processes, arrange and coordinate identified services, and provide follow-up to ensure clients connect with appropriate services. Additionally, the role acts as a liaison for Cyber Access and Swope Health Maple Woods services, assists with primary and specialty care needs, participates in post-hospital discharge planning, and facilitates multi-disciplinary team reviews. The position requires the use of various health information tools, outreach for specific programs, appointment scheduling assistance, and phone outreach to engage clients. The Care Coordinator will also run status reports, provide consultation on benefits and resources, maintain productivity expectations, complete progress notes, coordinate with other teams, and stay knowledgeable about agency technology. Support for marketing and public awareness initiatives, coordination of outreach events, and maintenance of required training are also key aspects of the role. A valid driver's license and reliable transportation are necessary, and other duties may be assigned by the supervisor.

Requirements

  • High School Diploma required
  • Active driver's license and minimum state-required auto-insurance
  • 1 to 3 years minimum work-related experience required
  • Knowledge of behavioral health diagnosis and symptoms
  • General knowledge of MoHealthNet, social security and other benefits, community resources and/or the willingness to research as needed
  • Intermediate to Advanced knowledge of Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, and Publisher)
  • Excellent communication skills, both written and verbal
  • The ability to use and troubleshoot basic business equipment
  • The ability to interact with a wide range of individuals
  • Strong ability to multi-task with a high level of accuracy and timeliness in project output

Nice To Haves

  • Bachelor’s Degree in related field, preferred
  • 3+ years minimum work-related experience preferred
  • CPR/First Aid certification

Responsibilities

  • Assist clients in applying for MoHealthNet and/or other benefits.
  • Maintain knowledge of Missouri Medicaid coverage/guidelines and community resources.
  • Assist with benefits applications and track their progress.
  • Provide outreach to engage consumers for MoHealthNet applications.
  • Educate consumers about MoHealthNet benefits and the application process.
  • Arrange and coordinate identified services and provide follow-up.
  • Act as the Cyber Access liaison with the Division of Behavioral Health.
  • Act as the secondary liaison to Swope Health Maple Woods services.
  • Assist consumers in accessing primary and specialty care needs.
  • Participate with post hospital discharges for individuals enrolled in HCH.
  • Facilitate multi-disciplinary teams’ reviews of monthly care management and hospital admission reports.
  • Document and notify appropriate team members of Hospitalization/ED admission.
  • Use health information tools (AvatarNX, CareManager, Cyber Access, KHIN, CIMOR and other designated systems).
  • Assist with DM3700 outreach.
  • Provide outreach on appropriate HCH, Swope PCP, and Tobacco Cessation referrals.
  • Assist with Nurse Care Manager appointment scheduling and tracking individuals’ services.
  • Provide phone outreach with identified clients who have not been engaging in services.
  • Run weekly status reports to track client data and identify client trends.
  • Provide consultation to other agency departments on benefits, community resources, and updates.
  • Maintain productivity expectations per agency standards.
  • Complete progress notes in AVATAR within 72 hours of services provided.
  • Coordinate with other organization teams to promote consumer wellness and connection to resources.
  • Maintain knowledge of agency technology programs and/or systems needed to do the job and use them effectively.
  • Track and assimilate data used in coordination of care.
  • Support marketing and public awareness initiatives with community involvement and promote Beacon programs and services.
  • Coordinate outreach events, presentations, and community education programs.
  • Maintain required training and certificates.
  • Attend and participate in bi-weekly team meetings.
  • Maintain a valid driver's license and reliable transportation.
