Job DetailsJob Location: Lincoln County - Branchland, WV 25506Position Type: Full TimeEducation Level: 4 Year DegreeSalary Range: $20.87 HourlyTravel Percentage: Up to 50% Job Category: Nonprofit - Social ServicesPrimary Responsibilities and Duties Screening, Assessment, and Engagement Establishes and maintains professional relationships with individuals, families, referral sources, and community partners. Completes initial and ongoing assessments in accordance with program, payer, and regulatory requirements. Identifies risk factors, including suicidal or homicidal ideation, and ensures immediate intervention, documentation, and follow-up in accordance with organizational protocols. Utilizes standardized screening tools (e.g., PHQ-9, SDOH assessments) as required. Care Coordination Coordinates services across healthcare, behavioral health, housing, employment, and social service systems. Facilitates communication among providers, natural supports, and community resources. Schedules appointments and assists with transportation coordination to reduce barriers to care. Participates in treatment team meetings and case reviews. Maintains timely, accurate, and compliant documentation in the electronic health record (EHR). Service Delivery and Support Provides targeted case management and supportive interventions consistent with scope of practice. Supports individuals in navigating systems of care and adhering to treatment recommendations. Promotes engagement in ongoing services and continuity of care. Advocates for individuals to ensure access to appropriate services and resources. QualificationsPrimary Attributes Professional & Technical Knowledge: Possesses the equivalent of formal training in behavioral health, usually in the form of a major in Psychology, Social Work, Counseling, or Criminal Justice as part of a Bachelor’s Degree program. Continuing Education: Maintains current knowledge of standards of care and practices, typically acquired through continuing education. Licenses & Certifications: A valid state-issued driver’s license is required. Technical Skills: Prepares basic correspondence and simple reports in Microsoft Word. Uses Microsoft Excel to create tables and simple displays of information. Creates basic presentations in Microsoft PowerPoint. Demonstrates necessary proficiency with all electronic clinical systems, including EHR and scheduling systems, in use at the Company. General Development: Displays basic organizational skills to self-prioritize and organize own work. Determines effective responses to situations encountered within established precedent. Able to effectively communicate opinions drawn from conclusions using inference and logic. Exercises tact and diplomacy in resolving mild conflicts or disagreements. Work Environment: Work is performed in community-based settings, including client homes and partner locations. Requires frequent travel within the service area. Physical activities may include walking, standing, climbing stairs, and occasional lifting of materials.

Requirements
