The Care Coordinator is responsible for coordinating the care for all ROWI clients. This role involves managing individualized services such as orientation, transportation, doctor visits, and scheduling. The Care Coordinator ensures programs and services are implemented at the highest standards and clients receive the highest level of care. They are also responsible for client supervision, facilitating groups, conducting client orientations, ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation, administering urine analysis, assisting with facility cleanliness, and coordinating daily client lunches. The role requires serving as a role model, maintaining confidentiality, complying with the dress code, completing tasks accurately and on time, and attending staff meetings. Additionally, the Care Coordinator must maintain clear boundaries, set limits, and manage clients when direct supervision is needed, ensuring appropriate and professional interactions. All EMR documentation must be submitted within 24 hours.
