About The Position

The Care Coordinator is responsible for using sound professional judgment and best practices to ensure quality services are provided to the service population while maintaining compliance with established program standards. BCFS Health and Human Services, established in 1944, is a non-profit organization with locations and nationally doing noble work across Texas and the United States, specializing in community-based care for at-risk families and children (abused and/or neglected & domestic violence), prevention & early intervention services, housing for young adults aging out of the foster care system, mental health & life skill services, advocacy and case management coordination for survivors of human trafficking, medical programs for women who are pregnant or post-partum, and serving families who have a child with a disability or special needs by providing them with an opportunity to visit the world’s first theme park designed with individuals with special needs in mind.

Requirements

  • High school diploma or equivalent.
  • With a high school diploma, at least 3 years of professional work experience in a related field.
  • Maintain certificates required for the position including but not limited to First Aid and CPR.
  • Valid Texas Driver’s license with a clear driving record and reliable transportation.
  • Pass a pre-employment drug screen and random drug screens.
  • Provide proof or work eligibility status upon request.
  • Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
  • Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.

Nice To Haves

  • Bachelor’s degree in a related field.
  • With a Bachelor’s degree, at least 1 year of professional work experience in a related field.

Responsibilities

  • Conduct outreach, intake, enrollment, and orientation activities for eligible participants.
  • Establish and maintain a trusting and professional working relationship with participants by providing regular contact.
  • Complete comprehensive participant assessments to identify strengths, needs, barriers, and goals.
  • Develop and maintain individualized service plan in collaboration with participant.
  • Provide case management and non-clinical mentoring services to participants.
  • Provide service planning and coordination, resource and referrals, basic needs support, and/or other support services to families on caseload.
  • Conduct systematic follow-ups for all families.
