The Care Coordinator provides service coordination to individuals with various disabilities, needs, and backgrounds. The Care Coordinator is vital to helping people with disabilities and their families navigate the health insurance system while promoting self-determination and a person-centered approach. A Hammer Care Coordinator provides services to individuals, ages 18-64, enrolled in a Special Needs Plan through a Managed Care Organization (Medica and/or UCare). The Care Coordinator supports individuals across the state of Minnesota to answer questions about their Medical Assistance health insurance plan, conducts assessments of address needs, and service planning around the individuals’ needs, goals, and preferences.
