About The Position

The Care Coordinator provides service coordination to individuals with various disabilities, needs, and backgrounds. The Care Coordinator is vital to helping people with disabilities and their families navigate the health insurance system while promoting self-determination and a person-centered approach. A Hammer Care Coordinator provides services to individuals, ages 18-64, enrolled in a Special Needs Plan through a Managed Care Organization (Medica and/or UCare). The Care Coordinator supports individuals across the state of Minnesota to answer questions about their Medical Assistance health insurance plan, conducts assessments of address needs, and service planning around the individuals’ needs, goals, and preferences. Requirements

  • Must have a reliable vehicle and adequate driving record as this position requires travel to in-person visits at client homes
  • Proficiency with Microsoft Office Products (Word, Excel, Outlook)
  • Ability to maintain tracking systems for multiple tasks to meet varying deadlines
  • Excellent written and verbal communication, with clients and other professionals
  • Flexibility to adapt to frequent process changes
  • Self-motivation to complete tasks independently
  • Willingness to ask questions
  • Experience, ability, and commitment to working with individuals and teams that are mixed across lines of difference such as race, gender-identity, sexual orientation, religion, ability, age, class, and immigrant status
  • Must be willing and able to work flexible hours/days, reflective of the dynamic schedule of the organization

Nice To Haves

  • Licensed Social Worker, Registered Nurse, Public Health Nurse, or Independently licensed mental health professional, preferred
  • Experience in disability services and/or the medical field preferred
  • Ability to demonstrate patience with individuals experiencing difficult circumstances

Responsibilities

  • Conduct assessment activities to determine the needs and strengths of each individual
  • Take person-centered approach to identify the needs and preferences of individuals on your caseload
  • Find resources to meet the unique needs of each individual
  • Complete referrals for benefits and connect individuals to in-network healthcare providers and services
  • Remain current on benefits and coverage available for applicable health plans
  • Maintain regular contact with interdisciplinary team ensure quality support
  • Identify goals, needs, and preferences of the individual
  • Develop support plan that integrates all necessary policy components
  • Update plan as necessary throughout the year
  • Promote health, safety, and well-being
  • Encourage independence and respect the right for individuals to take risks
  • Assist individuals in expressing their preferences and having their voice heard
  • Offer an in-person assessment to all individuals annually, either in their home or other community locations
  • Document case notes in electronic record system
  • Maintain case notes for all work throughout the day; must include a detailed, subjective narrative of all tasks completed
  • Ensure integrity in all documentation completed
